Deactivation Request Form

I , am an authorized representative of the Customer listed above. I/We understand that our company is responsible for payment of all charges and services up to and including the date shown above. I/We agree to pay for any delayed Inmarsat/Iridium call traffic received by Satcom Direct for our satcom usage, provided that the usage occurred during our ownership of the satcom as defined by our written notification to Satcom Direct via activation request and this subsequent deactivation request. I/We agree to pay for any and all call traffic received at Satcom Direct from the ground earth stations, including call traffic generated through equipment testing, equipment failure or automatic updates from our cabin information/weather update system.
